DSRIP Weekly Update-July 22, 2014
DOH issued new information this week regarding Safety Net Providers. In addition, don't forget to submit comment on Project Advisory Committtees, and don't miss the two fantastic webinars this week.
Safety Net Provider Updates
DOH has issued updated information regarding those entities that meet the Safety Net definition for the purposes of participating in the Delivery System Reform Incentive Payment (DSRIP) program. The updated list includes adult care facilities (ACFs), assisted living programs (ALPs), licensed home care services agencies (LHCSAs), and “other” providers, which includes multiple provider types including hospice and adult day health care (ADHC). The site also provides updated lists of nursing home, certified home health agency (CHHA) and long term home health care program (LTHHCP) safety net providers. We are pleased to see that many members were successful in their appeals process. Click here to view the lists.
We appreciate this additional information as we have been working to persuade DOH to be more comprehensive in their identification of Safety Net providers. We have been urging DOH to address this issue and ideally avoid requiring such a significant appeals process. A few weeks ago, we had submitted a memo to DOH with our colleagues at other associations on this issue.
It is important to note that providers that do not meet the Safety Net definition can still participate in DSRIP; however, such providers are limited to receiving no more than five percent of the funding for the project. It should also be noted that DOH plans to reopen the process before the Project Plan is due in December. Lastly, a separate form will be posted in the near future to collect information regarding the Vital Access Provider designation, which differs from the Safety Net Provider designation.

Bluestone has developed a care coordination model for seniors and people with disabilities throughout Minnesota, serving nearly 2,000 people with disabilities in the community. This webinar will discuss Bluestone's approach to care coordination, how they collaborate with their partners, and their impressive outcomes. There will be opportunities for questions.

MJHS and Maimonides Share Protocols for Improving Care Transitions

LeadingAge NY member Metropolitan Jewish Health System (MJHS) and Maimonides Medical Center (MMC) have partnered to improve care coordination for patients and families in their communities. They will present and discuss two programs:
Direct Home Care Admission Teams in the Emergency Department (ED): Through a coordinated protocol, patients in MMC's ED deemed ineligible for hospitalization, yet appropriate for home care services, may be admitted "directly" to MJHS' certified home health program. By staffing an MJHS home care intake nurse in MMC's ED, this transition can occur smoothly and efficiently, resulting in patients being directed to the most appropriate type of care expediently and reducing gaps in the critical follow-up care needed to keep patients safe and well at home.
Heart Failure Management Between Settings Results in Better Patient Outcomes and Lower Use of Hospital Care: MJHS' The Joint Commission Disease-Specific Care Certification program for heart failure provided the basis for a collaborative initiative between MJHS and MMC to establish care protocols for heart failure patients that include components of disease-specific care, patient/family education and self-care, and communication between providers and with patients. MJHS and MMC will describe the tools and protocols developed, implementation, and the outcomes, such as reductions in hospital readmissions.
Reminder: Project Advisory Committee: Opportunity to Provide Comment
As part of the DSRIP Design Grant Application process, each PPS was required to submit a Project Advisory Committee (PAC) proposal that outlined a stakeholder committee to advise them in development of their DSRIP Project Plan application (due Dec. 16). DOH is seeking public comment on each individual PPS PAC proposal, all of which are available on the DSRIP Design Grant web page as part of each Design Grant application. The PAC proposal is included in Section 10 of each application (the last tab in the Excel file).
Comments regarding the PPS PAC proposals are due to the state by July 28 at 5 p.m. Please be sure to reference which PPS PAC proposal you are providing comment for, and submit your comments to both DOH at dsrip@health.state.ny.us, as well as the lead point of contact for the PPS. The email address for the lead point of contact for each PPS is included in Section 1 of each application, and is also available on the Design Grant Application summary sheet on the Design Grant Applications page.
