CMS to Host Teleconference on Community-Based Care Transitions Programs

The Centers for Medicare and Medicaid Services (CMS) invites you to join a teleconference on June 13, 2014, 12-1 p.m. to learn about three evidenced-based social work Community-based Care Transitions Programs (CCTP). The CCTP is a CMS initiative, and the projects stemming from it are relevant to many of the initiatives currently underway on both the federal and state level. The CCTP was mandated by the Affordable Care Act and provides funding to test care transitions models for high risk Medicare beneficiaries. These models are relevant for organizations considering participating in a project under the Delivery System Reform Incentive Payment (DSRIP) program, as care transitions are featured prominently in the DSRIP Project Toolkit. In fact, the program highlights how the models improve transitions to reduce hospital readmissions, and features the use of the INTERACT tool, specifically highlighted in the DSRIP Toolkit as a project.
