LeadingAge NY to Distribute Universal Settlement Documents
To help expedite the final steps of the Universal Settlement process, LeadingAge New York is assisting the Department of Health (DOH) to distribute settlement documents. Nursing home administrators should expect to receive a packet of materials by e-mail from LeadingAge NY this week. Please note that whether you attend a signing event or not, the state is seeking to collect signed agreements in a very short timeframe.
Nursing homes are invited to attend one of the Universal Settlement signing events listed below to sign and submit their final settlement release documents. While homes are not required to attend as long as they submit the signed documents by the Dec. 5th due date, DOH is encouraging attendance to ensure the process is completed fully and quickly. The dates, locations and hours are as follows:
New York City
Nov. 23rd, 24th and 25th from 9 a.m. to 5 p.m. at 90 Church St, 14th Floor, Conference Room 4 A/B New York
(for homes in New York City, Long Island and the following counties: Orange, Putnam, Rockland, Westchester)
Rochester
November 30th from 8 a.m. to 5 p.m. at 335 East Main Street (DOH Offices), First Floor video conference room, Rochester
(for homes in the following counties: Allegany, Cattaraugus, Cayuga, Chautauqua, Chemung, Cortland, Erie, Genesee, Jefferson, Livingston, Monroe, Niagara, Onondaga, Ontario, Orleans, Oswego, Schuyler, Seneca, Steuben, Tioga, Tompkins, Wayne, Wyoming, Yates)
Albany
Dec. 3rd from 8 a.m. to 5 p.m. at Empire State Concourse, Meeting Room 5, Albany
