Final NHQI Score Sheets Available

(Dec. 16, 2025) The Department of Health (DOH) has finalized the 2024 Nursing Home Quality Initiative (NHQI) scores and has posted the final facility-specific score sheets on the Health Commerce System (HCS). The associated Dear Administrator Letter (DAL) which provides instructions for accessing the reports, outlines the methodology for calculating each measure, and provides insight on interpreting the scores is available here. The information will be posted to public DOH websites in the coming weeks.

As a group, LeadingAge NY members have customarily outperformed average statewide scores. This is important because NHQI scores are the basis for the redistribution of $50 million in Medicaid funding, with those in better quintiles seeing a net bonus, while those in worse quintiles face a net reduction. While DOH has not indicated when associated payment adjustments will be made, last year those occurred in March. We will keep members posted.
