DataPoint: County Healthcare System Performance

Suffolk, Rensselaer, Franklin, Cortland, Orleans and Cortland counties have higher rates of Medicaid avoidable hospitalizations for adult chronic conditions than other counties in their respective regions. Allegany, Oswego, Yates, Warren and Saratoga have low rates. This 2011-12 data, as well as an entire series of avoidable hospitalization by condition, has been posted here by DOH to assist in DSRIP project planning.    

Two chartbook series are available: one on avoidable hospitalizations, the other on how often recommended care was received. Each chartbok is organized by individual condition or treatment and by region. The regions used are the eleven defined for DSRIP purposes. Each region's data is shown in comparison to statewide figures and include individual county rates. Maps for each measure provide color coded indicators of the rate for each zip code within the region.    

While the purpose of the chartbooks is to assist potential DSRIP performing provider networks with better understanding of the patterns of health outcomes and healthcare within each region of the state to assist with network formation, it provides good insights as to where specific improvements in care may be needed.
