State Announces Transformation Grant Programs

Last week, the NYS Department of Health (DOH) and the Dormitory Authority of the State of NY (DASNY) announced the availability of funds for three opportunities under the Statewide Health Care Facility Transformation Program (SHCFTP) IV:

• SHCFTP IV: Applications due March 26, 2024, 4 p.m.; Questions on Request for Applications (RFA) due Feb. 20, 2024
• SHCFTP IV for Residential and Community-based Alternatives to the Traditional Model of Nursing Home Care: Applications due April 9, 2024, 4 p.m.; Questions on RFA due Feb. 14, 2024
• SHCFTP IV and V for Health IT, Cybersecurity, and Telehealth Transformation: Applications due March 13, 2024; Questions on RFA due Jan. 26, 2024
