General Transformation Grant RFA Deadline Extended to April 10, 2024
The Department of Health (DOH) has extended the deadline for applications for the Statewide Health Care Facility Transformation Program (SHCFTP) IV Request for Applications (RFA) #20244 to April 10, 2024 at 4 p.m.
Please note that this is the “general” transformation grant RFA (not the cybersecurity/technology RFA or the alternative nursing home RFA). DOH also made a change to one of the Frequently Asked Questions (FAQs) pertaining to this grant:
Q: Under Organizational Capacity, it says: Describe the Eligible Applicant’s organization by answering each of the following questions specifically below:
d. provide the name, title, salary of all employees in nonclinical titles whose salaries are $250,000 or more.
This appears to be a new item from prior RFPs. Is this as simple as submitting the organizations 990? Any particular reason to add this item?
A: Applicants should provide the name (if applicant wants the name to remain confidential then the employee name does not need to be reported) [emphasis added], title and salary of all employees in nonclinical titles whose salaries are $250,000 or more in response to Program Specific Question 3. Organizational Capacity, question d. Applicant should also a copy of their 2022 IRS 990 Form, if not exempt. All pages must be combined into a single PDF no larger than 10MB and uploaded as Attachment 8 via the Grants Gateway in the Pre-Submission Uploads section of the online application. Refer to Section V.A. Application Format/Content, Program Specific Question 6. Eligible Applicant Financial Stability.
