Alzheimer's Caregiver Support RFA Re-Issued for Central New York
The Department of Health (DOH) has re-issued a request for applications (RFA), under its Alzheimer’s Disease Caregiver Support Initiative, covering the Central New York area. This initiative will provide one regional contractor with up to $1 million for the first 8-month period and $1.5 million for each succeeding year over a 4 year, 8 month period (for a total of $7 million). The successful applicant must provide evidence-based or evidence-informed support services to caregivers of individuals with Alzheimer’s Disease and other dementias (AD/D), including all of the following core services:
- Care Consultation
- Family Consultation
- Support Groups
- Caregiver Education and Training Programs
- Respite.
In addition, at least one of the following evidence-based or evidence-informed caregiver support services should be proposed:
- Access Services (e.g. transportation, GPS locators, monitoring systems, etc.)
- Caregiver Companions
- Volunteer Care Support Teams
- Caregiver Wellness Programs
- Environmental Skill-Building and Training
- Joint Enrichment Opportunities
- Technology-based Services.
Applicants may partner with one or more organizations and may subcontract components of the project to other organizations. An optional letter of interest was due on Nov. 9th, and questions are due Nov. 13th. The deadline for responding to the reissued RFA is Dec. 16th at 6 p.m.
