Current Grants and Grant Opportunities Transformation Grant Funding Available; Deadlines for Questions Approaching!: Funding is available under three Statewide Health Care Facility Transformation Program grants. The deadline for questions pertaining to the Health IT-Telehealth grant is Jan. 26^th; the deadline for questions on the Alternatives to the Traditional Model of Nursing Home Care grant is Feb. 14^th; and the deadline for questions on the general Transformation grant is Feb. 20^th.
