The Moving Forward Coalition is seeking nursing homes to pilot a newly developed person-centered planning toolkit and training. The Coalition is working with Kansas State University to launch the toolkit in selected facilities.

The new model seeks to expand access to technology-supported care for chronic conditions through an outcome-based payment arrangement. Applications are due April 1^st.
