Please Complete Nursing Home Critical Asset Survey

All nursing homes should have received notification from the Department of Health (DOH), Office of Health Emergency Preparedness (OHEP) about the need to complete and submit, by the close of business on Oct. 14, 2016, their Critical Asset Survey.

The revised Nursing Home (NH) Critical Asset Survey is part of an ongoing redesign of the content and format of several DOH surveys, including previous Critical Asset Surveys, Coastal Storm initiatives, and SHARPS surveys. This new survey will consolidate information from multiple sources into one streamlined repository of data, with the capability to share this data with other Health Commerce System (HCS) applications.

The NH Critical Asset Survey is comprised of a combination of Planning AND Response topics in a Baseline form that will be administered to all healthcare sectors (hospitals/nursing homes/adult care facilities/community health centers). The Baseline form will be supplemented by sector-specific forms as needed to complete the process.
