Important Phase 2 Resources Now Available

Through a partnership with Pathway Health, LeadingAge NY has posted a series of resources, including policies and procedures, training tools, checklists, and assessment tools, related to the nursing home Phase 2 requirements. In addition to the Facility Assessment, which was posted last week, LeadingAge NY has posted resources for Antibiotic Stewardship, Baseline Care Plan, Infection Control, Medication Management, Psychotropic Medication, Self-Administration of Medication, and Smoking.

It is anticipated that additional resources will be posted shortly. These will include Behavioral Health, Staff Competency, Transfer/Discharge, Emergency Preparedness, QAPI, Abuse Prevention, and Resident Rights.
