Collaborative Drug Therapy Management

In the Dec. 2, 2015 NYS Register (page 8-12) there is a Proposed Emergency Rule that would expand the current CDTM Demonstation Program. The proposed amendment to the Regulations of the Commissioner of Education is necessary to implement Chapter 238 of the Laws of 2015, which amended the 2011 Education Law section 6801-a in relation to the Collaborative Drug Therapy Management (CDTM) Demonstration Program.  This program impacts physicians and pharmacists working under the auspices of a teaching hospital, by extending the CDTM program for an additional three year period and expanding CDTM to general hospitals and nursing homes with an on-site pharmacy staffed by a licensed pharmacist.

Collaborative Drug Therapy Management (CDTM) provides quality review of the performance of clinical services by a pharmacist relating to the review, evaluation and management of drug therapy to a patient, who is being treated by a physician for a specific disease or associated disease states.  The review is conducted in accordance with a written agreement or protocol with a voluntarily participating physician and under the policies, procedures, and protocols of the facility.

CDTM requires a written agreement and development of protocols between the physician, facility and pharmacist.  It would allow the pharmacist pursuant to an order by the physician to adjust the drug strength, frequency and/or route of administration of a medication.
