CMS Releases Updated MDS 3.0 Quality Measures User's Manual v18.0 Effective Jan. 1, 2026

(Dec. 2, 2025) The Minimum Data Set (MDS) 3.0 Quality Measures (QM) User’s Manual Version (v) 18.0 and associated files are now available for download here. Changes include the respecification of the Long Stay Antipsychotic Measure and a risk adjustor modification to the function measures based on the removal of item O0400 from the MDS version 1.20.1 and the Skilled Nursing Facility (SNF) QM User’s Manual V7.0 effective beginning Oct. 1, 2025.
