AANAC Article Features LeadingAge NY ProCare Consultant

In a July 23, 2014 article on the American Association of Nurse Assessment Coordinators (AANAC) website, Michelle Synakowski, LeadingAge NY policy analyst and ProCare consultant, is prominently featured in an article entitled Survival Tips for Late or Missed PPS MDSs.  Michelle, one of only a handful of AANAC Master Teachers in the United States, has presented nationally and has helped many LeadingAge NY members with their MDS process, and coding and care planning issues. 

Currently she is involved on the ProCare Quality Pool Metrics Review where she is helping members to improve their quality measures, avoidable hospitalizations and survey results to enable them to take advantage of the NYS Quality Pool funding awards.  As a result of her recent work in this area where she discovered an error in the Quality Measures for falls, NYSDOH acknowledged the error and recalculated the scoring, enabling 13 facilities previously denied funding to participate.
