CMS Updates Risk-Based Survey Resources

(September 15, 2026) Following our update on the nationwide implementation of the CMS Risk-Based Survey, CMS has released updated survey resources to support providers and surveyors as the new process takes effect.

The revised materials, available on the CMS Nursing Homes webpage under the Downloads section, include guidance and tools for the Risk-Based Survey, as well as resources for standard recertification surveys. Nursing home providers can review these updates to ensure survey readiness and familiarize staff with any changes to survey protocols.

As previously reported, CMS began nationwide implementation of the Risk-Based Survey on September 8, following the July release of QSO-26-14-NH.

CMS Updates Risk-Based Survey Resources DOH Issues Revised Nursing Home Incident Reporting Manual: DOH encourages nursing home administrators and others at the facility who are delegated responsibility for the submission of incident reports to review the manual and keep it readily available for reference.
