HANYS Recruiting for 2025 NYS Age-Friendly Action Community
(Oct. 22, 2024) With support from the NYS Department of Health (DOH) and the Institute for Healthcare Improvement (IHI), the Healthcare Association of NYS (HANYS) is preparing to launch its 2025 NYS Age-Friendly Action Community. This yearlong collaborative will guide participating hospitals, health systems, and post-acute settings in adopting IHI’s evidence-based 4Ms framework of Age-Friendly care to improve care for older adults.
HANYS will be hosting an Action Community recruitment webinar on Wed., Nov. 13th from 1-2 p.m., where it will introduce attendees to the basics and value of 4Ms care, the benefits of the Action Community, and the resources available to participating health systems as they work to earn official recognition from IHI. Dr. Kedar Mate, IHI president and CEO, will deliver the keynote address. Attendees will also learn how the Action Community is aligned with state and federal policies, including the Master Plan for Aging and the newly adopted Age-Friendly Hospital measure for the Centers for Medicare and Medicaid Services (CMS) Inpatient Quality Reporting Program.
