MLTC HIPAA Compliance

The Department of Health (DOH) is advising Managed Long Term Care (MLTC), Medicaid Advantage (MAP), and Program for All-inclusive Care for the Elderly (PACE) plans of the use of the Office for Civil Rights official form 960 for new enrollees, as part of your HIPAA compliance requirements.  Details on the requirements and a sample of the form are available under MLTC Policy 13.24: Authorization for Release of Protected Health Information.

DOH is advising plans that they should assist individuals in understanding the form.  A fully executed copy of the form must be maintained in the MLTC plan records.
