CMS Announces "First Batch" of Round Two Innovation Awards
The CMS Innovation Center announced, last week, twelve awarded projects under Round Two of Health Care Innovations Awards. These projects were deemed to be highly likely to drive health care system transformation and deliver better outcomes. The awards range from an estimated $2M to $18M over three years. CMS has indicated that the final batch of awards will be announced in the coming months.
New York State was well-represented among the recipients. The New York City Health and Hospitals Corporation (HHC), North Shore Long Island Jewish Health System (NSLIJ) and the Icahn School of Medicine at Mt. Sinai (Mt. Sinai) were all selected. HHC will test an emergency department care management model that utilizes a multi-disciplinary team to assess and treat patients who present in the emergency department for an Ambulatory-Care Sensitive Condition (ACSC), providing ongoing support after discharge, including medication management, education and linkages with primary care providers. NSLIJ will implement the Healthy Transitions (HT) Program to improve late stage chronic kidney disease costs and outcomes by strengthening patient education and preparation for renal replacement treatment, increasing home dialysis and preemptive transplantation, improving home safety, reducing medication errors and hospitalizations, and providing dietary counseling, depression screening, and advanced directive counseling. The Mount Sinai project will promote the Mobile Acute Care Team (MACT) model, which deploys a multi-disciplinary team to treat patients requiring hospital admission for selected conditions at home. The intervention also includes interdisciplinary post-acute care services for 30 days after admission.
