LeadingAge Massachusetts and Long-Term Quality Alliance Announce Housing and Health Pilot
It was recently announced that LeadingAge Massachusetts and the Long-Term Quality Alliance (LTQA) are developing the Massachusetts Housing and Health Pilot. The goal of this pilot project will be to bring housing providers and health plans together to deliver coordinated services and supports to affordable senior housing residents. These services and supports will largely be delivered by on-site teams made up of an enhanced resident service coordinator and a wellness nurse. These professionals will work directly with participating health plans to ensure that seniors in affordable housing are given the best opportunity to remain healthy and age in place. To learn what the members of New York State government, housing providers and developers, and services providers are doing to create models of affordable housing where seniors may receive the services they require to age in place, please attend the Second Annual Symposium on Healthy Senior Housing, presented by LeadingAge NY and Enterprise Community Partners. The event will take place on Oct. 19, 2017 in Albany, NY at the all-new Albany Capital Center. Leaders in New York State government, as well as innovators in the fields of developing housing with services and integrating services into housing models, will be presenting at the Symposium.
