Wage Parity Compliance Forms Posted
The Department of Health (DOH) has posted the wage parity forms that must be used by managed care and home care providers to be in compliance with the wage parity law.
As a reminder, this is year two of the implementation of the Home Care Worker Wage Parity law in Nassau, Suffolk and Westchester counties and year three implementation for New York City that begins on March 1, 2014. Forms certifying compliance with the law must be submitted via email to HCWorkerParity@health.state.ny.us by March 1, 2014. There are four attachments for either managed care organizations, certified home health agencies, long term home health care program providers or a licensed home care agency.
Please note the entities that contract with a LHCSA are required to obtain a written certification on a quarterly basis from the LHCSA. The forms remain with the contracted entity and are not sent to DOH.
There is no space on the forms for a date but please enter the date after your signature.
