Early Medicaid Health Home Lessons

The Center for Health Care Strategies will host on webinar on April 24, at 2:00 p.m.. to discuss lessons learned from implementing the health home model. The speakers, who are from Missouri and New York, will share their first-hand experiences and perspectives. The agenda will include:

• Overview of Lessons from Early Adopters
• Frontline Perspectives on Health Homes
• Moderated Q&A/Discussion

To date, 15 states have 22 approved state plan amendments to implement Medicaid health homes. The early "adopter" states are Iowa, Missouri, New York, North Carolina, Oregon and Rhode Island.
