Care Management for All Transition Summary and Timeline - October 2012

DOH posted the most critical MRT initiative for the transition of as many patient populations and services as possible to care management. This transition is called Care Management for All (CMA). The summary for CMA, which started last year, will take several years to complete. The Care Management for All Transition Timeline provides an overall summary, timeline for the populations’ transition and a timeline for the benefits transition. The timeline also includes the populations and services not transitioning.
