LHCSA Statistical Report Due This Friday, Nov. 16th
An urgent reminder: the Licensed Home Care Services Agency (LHCSA) Statistical Report is due this Friday, Nov. 16th. The LHCSA Statistical Report includes New York State’s new annual registration requirement for all LHCSAs and Assisted Living Program (ALP) LHCSAs. If you have not already done so, you must fill out and submit these reports immediately.
Traditional LHCSAs must fill out the LHCSA Statistical Report. LHCSAs that EXCLUSIVELY serve residents in an ALP are only required to submit the 2017 ALP LHCSA Statistical Report on the HERDS application of the Health Commerce System (HCS) AND the following parts of the 2017 LHCSA Statistical Report:
- Form LSR1 – Agency Information Form
- Form LSR2 – Patient Form – the first three questions only
- Form LSR7 – County Form – Unduplicated Patient Count question only
- Forms LSR9WFA, LSR9WFB, LSR9WFC – Workforce forms A, B, and C
- Form LSR10 – LHCSA Registration Form.
If your LHCSA serves ALP residents AND also serves patients in the community outside of the ALP, complete and submit all forms of the 2017 ALP LHCSA Statistical Report and the 2017 LHCSA Statistical Report.
Penalties will be imposed for LHCSAs that do not successfully submit the report and will begin after midnight on Nov. 16th.
Fines and penalties will be imposed immediately following the deadline:
- A $500 fine will be assessed for each month, or part of a month, that the organization is not registered.
- Agencies that are not registered by the deadline will not be allowed to operate until they become registered.
- LHCSAs not registered by Jan. 1, 2019 will not be able to provide services, bill, or be reimbursed for services.
- Agencies that fail to register timely for two years, not necessarily consecutive, may have their licenses revoked.
After submission, you should receive a confirmation email indicating successful submission of a complete 2017 Statistical Report. Please contact hcstatrpts@health.ny.gov if you have submitted your report and not received such a confirmation.
