2023 LHCSA Registration Short Form Due Dec. 31st

LeadingAge NY members should be aware that the Department of Health (DOH) has posted the Licensed Home Care Services Agency (LHCSA) Registration Short Form 2023 for all LHCSAs to complete by Dec. 31, 2022.

This is an annual registration form required by law. Both traditional LHCSAs and Assisted Living Program (ALP) LHCSAs are required to complete it. LHCSAs that are not registered may not provide services or receive reimbursement for services. Registration will not be deemed fully effective until the LHCSA Statistical Report is submitted by the deadline of close of business Feb. 28, 2023.

Directions for using the LHCSA Registration Short Form:

• Log on to the Health Commerce System (HCS).
• Open HERDS in your "My Applications."
• If you do not have HERDS listed under "My Applications":
  • Click on the "My Content" tab on the HCS menu bar.
  • Click on "All Applications."
  • Click on H.
  • Locate HERDS (Health Electronic Response Data System).
  • Click on the green ball with the + (plus sign) on the righthand side.
  • Navigate back to the HCS home page.
  • HERDS should now be listed in your "My Applications."
  • Open HERDS.
• Click on Data Entry.
• Select LHCSA Registration Short Form as the Activity.
• Enter your data.
• SAVE and SUBMIT your data.
• Make sure you select 2023 as the Report Year.

The LHCSA Statistical Report instructions and forms will soon be posted on the HCS, and that report must be completed by Feb. 28, 2023. ALP LHCSAs that serve only their residents must complete the ALP LHCSA Statistical Report. For ALP LHCSAs also serving individuals in the community, the general LHCSA Statistical Report must be completed.
