CMS GUIDE Model Participants Made Public

(July 9, 2024) The Centers for Medicare and Medicaid Services (CMS) announced almost 400 approved health care organizations building Dementia Care Programs (DCPs) as part of the Guiding an Improved Dementia Experience (GUIDE) Model, CMS’s new care and payment model for Medicare beneficiaries with dementia and their caregivers aimed to help individuals with dementia remain in their homes.

The Model will test an alternative payment arrangement for providers that deliver supportive services to individuals with dementia, including assessments, care plans, care coordination, and 24/7 access to a support line. Individuals with dementia and their caregivers will have access to an interdisciplinary team, including a trained care navigator and experienced clinician, who will help them access both clinical and non-clinical services. The Model will also help caregivers to access education, support, and respite services.

Lead GUIDE participants are Medicare Part B-enrolled providers/suppliers who are eligible to bill for Medicare Physician Fee Schedule services and agree to meet the care delivery requirements of the Model. Participating providers who do not meet the GUIDE care delivery requirements alone may contract with other Medicare providers/suppliers to meet the care delivery requirements.
