CMS GUIDE Models Delivering Dementia Care and Support in NY

(March 18, 2025) On July 1, 2024, the Centers for Medicare and Medicaid Services (CMS) launched the Guiding an Improved Dementia Experience (GUIDE) Model, a voluntary nationwide model test that aims to support people with dementia and their unpaid caregivers. The model will run for eight years. Established models started on July 1, 2024, and new models will start delivering care on July 1, 2025.

Under the GUIDE Model, providers deliver the following services:

  • Comprehensive Assessment and Care Plan
  • Caregiver support
  • Respite services provided either in the home or at adult day care programs
  • Coordination and support with meals, transportation, and more
  • Coordination with clinical and support services
  • Care navigation and 24/7 access to care team members to help with questions or support

CMS announced the 390 organizations participating in the model and published a GUIDE Participant List. Please visit the GUIDE Model webpage for additional information.
