CMS Announces MAHA ELEVATE Model, Will Issue NOFO in Early 2026

(Dec. 15, 2025) The Centers for Medicare and Medicaid Services (CMS) has announced plans to release a Notice of Funding Opportunity (NOFO) for pilot projects for disease prevention and health promotions in early 2026 under the new Make America Healthy Again: Enhancing Lifestyle and Evaluating Value-based Approaches Through Evidence (MAHA ELEVATE) Model. In general, the model will provide approximately $100 million to many types of eligible organizations, including senior living communities, to fund three-year cooperative agreements for up to 30 proposals that promote health and prevention for Original Medicare beneficiaries.
