Money Follows the Person Request for Applications
The Department of Health (DOH) released a Request for Applications (RFA) for the Money Follows the Person (MFP) program on Fri., Feb. 7, 2014. There are two components to the RFA:
- Peer Outreach and Referral - peer-based approach that reflects the characteristics of individuals who are interested in moving from an institutional setting to the community.
- Transition Centers Project - actively solicit, generate and accept referrals which may identify individuals who qualify for MFP.
Applicants can apply for one or both components of the RFA.
The purpose of the program is to continue promoting the opportunity through education and outreach for seniors and individuals with disabilities to have access to community-based services rather than institutional care. MFP was started in 2007 and has been extended until 2016 in the state. Originally it started in the Nursing Home Transition and Diversion (NHTD) waiver, then to the Traumatic Brain Injury (TBI) waiver and then in 2013 into the OPWDD Home and Community-Based Services (HCBS) waiver. MFP is also included in the Balancing Incentive Program (BIP). The State has indicated that MFP will expand into Managed Long Term Care (MLTC) plans and individuals with behavioral health and mental health issues.
