The goals of the initiative are to support the engagement of Area Agencies on Aging in health care partnerships and to address the social determinants of health of older adults.

This opportunity is for new NORC applicants or existing NORCs that would like to establish a new program. Funding is not available to supplement existing programs.

RFA abstract for expansion is now available. Applicants must register on Grants Gateway and complete needs assessment to apply later this summer.

NYSOFA will be issuing an RFA this summer to fund new NORC programs with approximately $3.2 million in additional funding provided in the recently enacted 2018-19 State Budget.

Questions on RFA are due to DOH by March 22^nd; application deadline is May 4^th.
