Extension for CHHA applicants - November 13, 2012

As a result of Hurricane Sandy, DOH extended the deadline for Certified Home Health Agency (CHHA) applicants who had received a 30-day extension to submit supplemental information. The Dear Administrator Letter provides a new deadline of Dec. 7, 2012. Additional information should be submitted to: Rebecca Fuller Gray, Director, NYSDOH, Division of Home and Community Based Services, 875 Central Ave., Albany, NY 12206.
