ICD-10 Readiness Survey

The Department of Health (DOH) will be transitioning to ICD-10 diagnosis and procedures codes for providers and payers covered by the Health Insurance Portability and Accountability Act (HIPAA) in October 2015. The transition to ICD-10 will not impact CPT codes.

DOH is interested in learning about providers’ readiness for the upcoming ICD-10 implementation and is asking that you complete and submit the following brief, eleven question, anonymous survey. This will help them design and target their communication and implementation efforts, as October 2015 is less than a year away.
