DOH Surveys Current and Former LTHHCPs for CFCO Readiness
The Department of Health (DOH) is surveying current and former Long Term Home Health Care Programs (LTHHCPs) for their interest and capacity to offer Community First Choice Option (CFCO) benefits. CFCO is an enhanced personal attendant Medicaid program that provides assistance with activities of daily living (ADL), instrumental activities of daily living (IADL), and certain health-related tasks through hands-on assistance, supervision, and/or cueing. CFCO services are to be offered statewide and across all disability populations. The survey is intended to identify entities that could provide or help arrange for the following CFCO services: Assistive Technology, Community Transitional Services, Environmental Modification, Home Delivered/Congregate Meals, Moving Assistance, and Vehicle Modification.
Please see the Department's Dear Administrator Letter (DAL) and Excel survey document. The survey is due Dec. 14th. Responses to the survey and questions should be directed to CFCO@health.ny.gov.
