2014 Annual Statistical Reports for Home Care and Hospice
The Department of Health (DOH) has posted to Health Commerce System (HCS) a Dear Administrator Letter (DAL) with the timeframe for activating and completing the 2014 annual statistical reports for Certified Home Health Agencies (CHHAs) and Long Term Home Health Care Programs (LTHHCPs), Licensed Home Care Services Agencies (LHCSAs) and hospices.

Attached to the DAL are the instructions for completing the surveys and submitting the data to DOH.
For the Assisted Living Program (ALP) – As before, a separate statistical report will be used to collect data from LHCSAs exclusively providing services to ALP residents.
According to the DAL, “Agencies not submitting all appropriate forms for these required surveys will be cited and subject to enforcement action."
