Institute for Healthcare Improvement to Launch Second Age-Friendly Health Systems Action Community
The Department of Health (DOH) informed LeadingAge NY last week that the Institute for Healthcare Improvement (IHI) is launching their second Age-Friendly Health Systems Action Community and is seeking additional participants.
This Action Community will begin on Mon., April 1st. It is a free, seven-month learning community that succeeds the first Action Community of teams from more than 75 systems across the nation working together to rapidly test and scale the Age-Friendly Health Systems 4Ms (What Matters, Medications, Mentation, and Mobility) Framework.
