Expectional Care Planning (ECP) is an interdiscliplinary approach that utilizes Standards of Care (SOC) to streamline the care planning process and improve consistancy in the medical record.   The tools and resources provided here were developed based on feedback from grant participant sites, monthly conference calls and individual consultations for the 2012-2013 Replication Project funded by the New York State Health Foundation.
