DOH Provides Resources for Changes in Preadmission Process for Transitional Adult Homes
The Department of Health (DOH) hosted and recorded a webinar on Jan. 31, 2022, which was repeated on Feb. 1, 2022, outlining changes to the preadmission process for Transitional Adult Homes (TAHs). Under the revised process, as described during the webinar, preadmission screening and Mental Health Evaluations will be conducted by an entity retained by the Department, CSRA State and Local Solutions LLC, which is a subsidiary of General Dynamics Information Technology (GDIT), and its subcontractor, Keystone Peer Review Organization Inc. (Kepro). TAHs will submit requests for screening and, when needed, Mental Health Evaluations directly to Kepro.
The Department’s Division of Adult Care Facility and Assisted Living Surveillance will shortly issue a Dear Administrator Letter identifying the date when such requests must be made to Kepro.
In the interim, effective immediately, TAHs may submit requests to Kepro.
To request that Kepro complete a preadmission screen:
- Send an encrypted email to TAH_Assessments@kepro.com, fax to 718-816-3560, or use Kepro’s online portal.
- Include the Psychiatric Services and Clinical Knowledge Enhancement System (PSYCKES) Consent Form signed by the prospective resident, which reflects the individual’s consent to CSRA State and Local Solutions LLC and designated staff of Kepro accessing their Medicaid data through the Office of Mental Health's PSYCKES. TAHs must also give a copy of the completed consent to the prospective resident.
Kepro will determine whether the individual is suspected to be a person with serious mental illness. Results will be provided within three business days of receipt of a complete screening request.
To request that Kepro complete a Mental Health Evaluation:
- Send an encrypted email to TAH_Assessments@kepro.com, fax to 718-816-3560, or use Kepro’s online portal.
- Submit the DOH Authorization for Release of Health Information (Form 5032). Include the individual’s information at the top of the form, include the name of the TAH in Boxes 5 and 6, and ask the individual to add the start date and initial two places in Box 8. The form must be signed and dated by the prospective resident, and the TAH must give a copy of the completed form to the prospective resident.
Kepro will provide the TAH with a copy of the completed Mental Health Evaluation.
