Reminder: 2012 ACF Census Report due Jan. 27

DOH has just issued the 2012 ACF Annual Census Report, which must be submitted on the Health Commerce System (HCS) by Jan. 27, 2014.  Based on feedback from LeadingAge New York and the provider community, DOH has significantly revised the census report.  We hope you find the report to be more user-friendly, and encourage feedback about your experience.

It is important to note that the census encompasses calendar year 2012, prior to adoption of the regulatory amendment on Jan. 16, 2013. Therefore, for the purpose of this census, the definition of serious mental illness does not apply and Transitional Adult Homes are not required to complete the Roster of Adult Home Residents for calendar year 2012.  In addition, while several individuals may enter data for this report, the Administrator must review the report prior to submission and “sign.”
