New ACF Quarterly Statistical Report Materials Available

As was reported last week, DOH has issued information regarding the new quarterly statistical report, which must be completed by all Adult Care Facilities (ACFs).  Additional information is now available on the HCS at the top of the ACF DALs 2013 Documents, entitled “Census Report.”  The materials can also be viewed here.  We recommend you review the DAL and four attachments carefully before beginning the report.

The report covers the second quarter of 2013 and is due Aug. 1.  It must be submitted electronically and the facility administrator must review the report and sign the attestation statement prior to submission.
