EQUAL Applications Due Feb. 12th

The application for EQUAL funding is available now on the Health Commerce System (HCS), with a very short timeframe for submission: applications are due by 5 p.m. on Wed., Feb. 12, 2014.  For your convenience, we have posted the Dear Administrator Letter (DAL) and all related attachments on this page. The DAL outlines examples of allowable expenditures. The application must demonstrate resident council support of the initiative.

We recommend that you review closely the conditions for participation. As was the case last year, facilities must have a Statewide Financial System (SFS) account to receive EQUAL funds. If you do not yet have an account, we recommend you start this process immediately. In addition, your census reports must be completed in order to receive EQUAL funding. Questions regarding EQUAL should be directed to DOH’s Nancy Nowakowski or Dorothy Persico at 518-408-1133.
