Don't Miss This Training on the Role of Case Management in ACF and Assisted Living Settings

LeadingAge NY is proud to partner with Frank Rose of RFR Consulting Group and Kathy Sindoni of Substantial Compliance Consulting Group to provide training for adult care facilities (ACFs) and assisted living (AL) on the role of case management. Both professionals have a wealth of experience in the field and are well-regarded educators and consultants; click here to learn more about them. Particular attention will be given during the training to the role of nurses who work in various ACF/AL settings. The training is a day long and will be held in four different locations throughout the state during the month of January.

Why Is This Important?

This training was developed by Rose and Sindoni because the role of case management in ACF/AL settings is critical to nearly every aspect of operations, and both the importance and complexity of that role is only growing. At the same time, the role of nurses who work in these settings, as case managers or in other roles, can be a source of confusion for many. This session will help to clarify how professionals such as nurses and social workers fit into the framework of ACF/AL settings and how their skills can be best utilized. Having a strong case management approach will also result in better outcomes for your residents and their loved ones, your organization, and your Department of Health (DOH) survey.

Who Should Attend?

While the focus of the training is on case management, nursing, and social work in ACF/AL settings, many can benefit from it, including:

  • Case managers
  • Nurses 
  • Social workers
  • ACF/AL administrators
  • Quality assurance professionals
  • Professionals new to ACF/AL, including those who applied for assisted living program (ALP) beds
  • Professionals who have worked in these settings and want a refresher on these issues, as well as guidance about new and upcoming requirements
  • ACF/AL providers who want to strengthen their performance on survey as it relates to case management and related issues

Variety of Credit Offerings for a Broad Audience

We are pleased to offer an array of credit options, given that this program appeals to a broad audience. The program offers:

  • up to seven hours of continuing education credit for ACF/AL professionals;
  • up to seven CEUs for nursing home administrators under its sponsor agreement with the National Association of Long Term Care Administrator Boards (NAB)/National Continuing Education Review Service (NCERS); and
  • up to seven contact hours for Licensed Master Social Workers (LMSW) and Licensed Clinical Social Workers (LCSW).
