DOH Provides Updated Forms for ACFs for Controlled Substances

The Department of Health (DOH) has provided LeadingAge NY with the current, fillable versions of two forms:

• Request for Approval of Disposal/Destruction of Controlled Substances (DOH-2340); and
• Controlled Substance Inventory Form (DOH-166).

The email address to submit the forms to is at the bottom of the DOH-2340.
