DOH Alert for Downstate ACFs

DOH has asked LeadingAge NY to inform Adult Care Facilities (ACFs) located in the Metropolitan Area Regional Office who correspond with the 90 Church Street, NYC office of the following:

Due to a recent upgrade in the phone service at 90 Church Street, there is a change in the Adult Care Facility fax number. Effective immediately, the new fax number is 212-417-2119. Please ensure all DSS-3123's and any other correspondence intended for the 90 Church Street Adult Care Facility office are faxed to this new number.  The old 212-417-5080 number is no longer in service.

This alert applies only to facilities corresponding with the Church Street office, and does not apply to those facilities who correspond with the Central Islip office.  Please feel free to contact the Metropolitan Area Regional Office at 212-417-4440 if you have any questions or concerns.
