All ACFs Must Complete QSIR by Oct. 30th

The Department of Health (DOH) has activated the 3rd Quarter Statistical Information Report (QSIR) on the Health Commerce System (HCS). All adult care facilities (ACFs) are required to complete the QSIR, which is for the reporting period of July 1, 2015 to Sept. 30, 2015. Additionally, facilities with a certified bed capacity of 80 beds or more, in which twenty percent or more of the resident population are persons with serious mental illness, are required to submit the Roster of Adult Home Residents. The report must be submitted via the HCS by Oct. 30, 2015.  Below are four documents to assist providers in completing the report; members are encouraged to review them carefully.

Members are reminded that DOH is requring providers be up to date on all required reports in order to have their EQUAL applications considered.
