ACFs and Nursing Homes Should Submit Critical Asset Survey
All adult care facilities (ACFs) and nursing homes should have received notification from the Department of Health (DOH), Office of Health Emergency Preparedness (OHEP) about the need to complete and submit, by the close of business on Oct. 14, 2016, their Critical Asset Survey on the Health Commerce System. The questions contained in this form pertain to the infrastructure and utilities at each facility. This data will be used during the current Hurricane Season for Planning and (potential) Response purposes.
For ACFs, this is a baseline survey and may be supplemented in the future with additional forms. For nursing homes, the Critical Asset Survey is part of an ongoing redesign of the content and format of several DOH surveys, including previous Critical Asset Surveys, Coastal Storm initiatives, and SHARPS surveys. This new survey will consolidate information from multiple sources into one streamlined repository of data, with the capability to share this data with other Health Commerce System (HCS) applications.
Questions regarding the content of this survey may be directed to the Office of Health Emergency Preparedness at 518-408-5163.
