ACF Members are Reminded that Financial Report is Due Jan. 6th
Members are reminded that the 2015 Financial Report is available for completion on the Health Commerce System (HCS). The deadline for submission of the 2015 ACF Financial Report for facilities using a calendar year (Jan. 1st – Dec. 31st) as their fiscal year is Jan. 6, 2017. Facilities using a fiscal year period that differs from a calendar year are required to file their report by the 15th day of the sixth calendar month following the close of their fiscal year.
Please note that the “Opinion of the Independent Accountant – Certification of Operation” page (AH) or the “Certification of Operator” (EHP) statement should be printed, signed, and mailed to DOH. Please see the DAL and Instructions document for more information and details.
