ACF Financial Report Available on the HCS

DOH has activated the financial report for adult care facilities on the Health Commerce System (HCS). Click here to view the Dear Administrator letter outlining the process and due date for submission. If you have any questions about how to access the application, or need further clarification on the overall report process, please send an email to DOH at HCBS-survey@health.state.ny.us, or call (518) 408-1847. For any questions specific to the Financial Reports, or for HCS account requests, please contact DOH's Nancy Nowakowski at (518) 408-1624 or via email at: acfinfo@health.state.ny.us (subject line: Financial Report).
