Reminder: ACF Annual Census Report Due May 16

The ACF Annual Census Report for 2013 is available on the Health Commerce System (HCS).  The report must be submitted electronically on the HCS by May 16.  For your convenience, the Dear Administrator letter and three attachments are posted on our website here.  This report satisfies the requirement for the fourth quarterly report of 2013.  Transitional adult homes will also receive communication directly from the Department of Health to update their rosters.
