ACF and Assisted Living Members Are Reminded of ACF Financial Report Due Date
(April 29, 2024) Adult care facility (ACF) and assisted living members are reminded that the 2022 ACF Annual Financial Report has been activated on the Health Commerce System (HCS). The Dear Administrator Letter (DAL) was posted in December 2023.
All ACFs that were in operation for six months or more during calendar year 2022, as well as ACFs that underwent a change of operator between related ACFs during calendar year 2022, must submit the report on the HCS.
In addition, the ACF Certified Public Accountant (CPA) Certification must be completed by all ACFs with 26 beds or more. See the below enclosures:
- Independent Report of Certified Public Accountant (CPA) Single Facility – Enclosure II
- Independent Report of CPA Consolidated Financial Report – Enclosure III
The Certification of Operation statement (Department of Health (DOH)-5780) must also be completed and submitted by all ACFs.
The deadline for submission of the 2022 ACF Annual Financial Report and the relevant certifications follows:
- For ACFs using a calendar year as their fiscal year, the due date is May 31, 2024. ACFs using a fiscal year period that differs from a calendar year are required to file their report by the 15th day of the sixth calendar month following the close of their fiscal year. If this falls before May 31, 2024, then the due date is May 31, 2024.
