ACF and AL Members Are Reminded That Applications for EQUAL Funds Are Due July 6th

All adult care facility (ACF) and assisted living members that serve recipients of Supplemental Security Income (SSI) or Safety Net benefits are reminded of the opportunity to apply for EQUAL funds. The application is due by 5 p.m. on July 6, 2018.

Members are encouraged to review their application before submission to ensure you have completed all parts. The Department of Health (DOH) will not accept late submissions, and an incomplete application will not be considered. Click here for more information regarding the opportunity and how to apply.
